Dental Insurance

As a courtesy to our patients, we will submit dental claims for you and accept assignment of benefits to receive payment directly from your insurance company. We ask that you bring all pertinent and current insurance information with you to your initial appointment. This information allows us to help determine your benefits. Patients are ultimately responsible for the discovery and understanding of the limitations of their insurance plan.

Payment of your insurance deductible and the percentage of treatment not covered by your insurance plan will be expected at the time services are rendered. Dental insurance is a contract between the patient, the employer, and the insurance carrier; not between the dentist and the insurance carrier.

Dr. Nicole Ferrara and Dr. Ted Mioduski, III, are in contractual agreement with Delta Dental as Premier providers. Although IGDNC may not be in contractual agreement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled.
